HOURS: DAILY, 7:00AM TO 11:00PM
Members of the public may request copies of records maintained by the Greenfield Police Department. You may request these records online or make a request at the Greenfield Police Department whenever the public lobby is accessible and open to the public at the hours listed above. Under most circumstances a completed report will be available within 10 business days.
ALL RECORDS ARE REVIEWED BY THE RECORDS SUPERVISOR OR DESIGNEE PRIOR TO RELEASE. THE COST FOR A REPORT IS $0.25 PER PAGE.
If law enforcement is called to the crash, then that agency will complete the report. If law enforcement does not file a report or does not respond to the scene, you will need to complete the Wisconsin Driver Report of Crash DT4002.
When does the law require a crash report? (One or more of the following must be applicable)
- Injury of a person
- Damage of $1,000 or more to any one person’s vehicle or property
- Damage of $200 or more to state or other government-owned property other than a vehicle

Accident reports are available for pick up at the Greenfield Police Department, 5300 West Layton Avenue, Greenfield, WI 53220 between 7:00AM and 11:00PM. PLEASE CALL PRIOR TO PICK UP (414-761-5330) to ensure report is complete and available for pick up.
For incident reports and/or other records please fill out the form below and submit the request either online or in person. Once received, this request will be forwarded to the records department for fulfillment and approval. The records clerk will notify you by phone when available for pick up.
